A new study appearing in Leukemia Research from researchers at the Institute for Myeloma & Bone Cancer Research seeking to better understand the phenomenon evaluated 379 samples from relapsed/refractory MM patients and found elevated levels soluble BCMA that were associated with reduced binding by an inhibitory antibody. The group thus suggested that free-floating protein act as a “decoys” that may contribute to diminished efficacy of anti-BCMA therapies.

One possible solution to the issue of circulating BCMA that has been proposed is to combine a direct antagonist, be it a CAR-T construct or antibody-based therapy, with an inhibitor of gamma secretase, an enzyme that cleaves transmembrane proteins and is believes to lead to increased levels of soluble BCMA.

Novartis is one of the companies that has moved aggressively in this area, partnering to test Ayala Pharmaceuticals’ AL102 in combination with the Swiss pharma’s CAR-T therapy. Juno Therapeutics (acquired by Celgene last year), also licensed a gamma secretase inhibitor from Eli Lilly in late 2017, though Amin noted that nothing has been heard about the programme in the years since.
