De seneste udmeldinger i mandags fra Gavi, EU og Africa CDC peger på markante strategiske skift i global sundhedspolitik - fra finansiering og donorafhængighed til øget selvstændighed og integration af sygdomsberedskab i nationale sundhedssystemer.

Why is investing in regional and local manufacturing capacity essential to strengthening health security, and what is the EU doing to support it?

Referat
I interviewet fremhæver Martin Seychell, Deputy Director-General i DG INTERNATIONAL PARTNERSHIPS (INTPA), at global sundhed ikke kun handler om akutte kriser, men også om at skabe bæredygtig velfærd og stabilitet i samfundet.
Han peger på, at selvom økonomisk vækst finder sted i mange lande, betyder vækst alene ikke meget, hvis den ikke omsættes til bedre sundhed og livskvalitet.
Et centralt fokus er at investere i regional og lokal fremstilling af medicinske produkter - vacciner, medicin, diagnostik osv. - fordi erfaringen fra COVID-19 viste, at store dele af verden var ekstremt afhængige af import, fx underforsyning i sub-Sahara Afrika og Latinamerika.
Ved at styrke produktionen regionalt reduceres sårbarheden over for afbrudte forsyningskæder, eksportstop og manglende adgang under kriser.
Han beskriver, at EU's tiltag retter sig mod både at hjælpe partnerlande med at udvikle egne kapaciteter og at samarbejde globalt for at sikre mere retfærdig adgang til medicinske modforanstaltninger.
Endvidere fastslår han, at selvom globale sundhedstrusler (pandemier, smitsomme sygdomme) er akut vigtige, må indsatsen også omfatte det, der foregår "imellem kriserne" - opbygning af robuste sundhedssystemer og forebyggelse.

INTPA samarbejder tæt med bl.a. HERA, Udenrigstjenesten (EEAS) og WHO for at styrke global modstandskraft, bl.a. gennem støtte til regional og lokal produktion af sundhedsprodukter i Afrika, Latinamerika og Asien.
INTPA står for Generaldirektoratet for Internationale Partnerskaber (Directorate-General for International Partnerships) og er en del af Europa-Kommissionen.
INTPA har ansvaret for EU's politik for internationalt samarbejde og udvikling. Målet er at bekæmpe fattigdom, fremme bæredygtig udvikling og støtte global stabilitet.

Ifølge briefingen har Africa CDC fortsat diskussioner med Bavarian Nordic om hvordan Tech Transfer kan finde sted fra Bavarian Nordic til Syd Afrika "funding is missing to support 3-4 mio. doses needed to support local manufacturing." ca. 41:00 - lyt med fra ca. 37:30 (Q/A) vedr. Vaccine Manufacturing in Afrika.
(Mon den mpox-stockpile Gavi vil etablere i sommeren 2025 kunne bidrage til etablering af local manufacturing?)
Der nævnes endvidere, at et møde finder sted i denne uge, hvor der skal tages stilling til at anbefale om PHECS skal fortsætte eller ophæves.
Mpox fylder mindre og mindre i briefingen - nu hvor mpox snart forventes at overgå til "primary healthcare" i de enkelte lande. Det fortælles, at tallene for mpox falder fortsat i Afrika. Mit indtryk er: når man ikke tester, finder man heller ikke noget.
Kolera er meget mere dødelig og fylder derfor forståeligt nok mere og mere i briefingen. Africa CDC anvender samme skabelon, der er bygget op omkring håndtering af Mpox, til at håntere nye udbrud, herunder kolera.

Skal Californien og andre stater have deres egen sundhedsmyndighed?
I kølvandet på kritik af amerikanske føderale sundhedsmyndigheder som CDC, især omkring håndtering af vaccinationer og offentlig kommunikation om sygdomsforebyggelse, har flere amerikanske delstater - med Californien i spidsen - taget initiativ til at skabe et regionalt sundhedssamarbejde. Dette samarbejde er formelt kendt som West Coast Health Alliance og omfatter bl.a. Californien, Oregon, Washington og Hawaii. Det sigter mod at koordinere evidensbaserede anbefalinger om vacciner, sygdomsforebyggelse og andre offentlige sundhedsindsatser uden nødvendigvis at være afhængig af CDC's udmeldinger.
