Unlocking the potentials of community health workers for effective control of mpox outbreaks in Africa
Since May 2022, multiple countries have reported clusters of monkeypox (mpox) virus infections. In response to the current upsurge of mpox in the Democratic Republic of the Congo and an increasing number of African countries, the Africa Centers for Disease Control and Prevention (Africa CDC) and World Health Organization (WHO) declared mpox as a Public Health Emergency of Continental Security and a Public Health Emergency of International Concern on the 13th and 14th,/sup> of August, 2024, respectively. Although community health workers (CHWs) were instrumental in controlling the 2014/2015 Ebola outbreak across West Africa, little attention is given to the potential importance of CHWs in the prevention and control of large-scale mpox outbreaks. Community health workers (CHWs) are often considered trusted voices in the community and can be critical to help raise awareness on mpox and deliver preventive messages in a sensitive and culturally appropriate manner. Moreover, CHWs can be instrumental in addressing common misconceptions and false rumours that hinder healthcare-seeking behaviours. Another adaptive role played by well-trained and adequately equipped CHWs is surveillance through door-to-door active case finding of mpox cases, which is crucial in reducing dissemination at community level. Effectively tackling vaccine hesitancy is essential to ensure herd immunity and enhance mpox response. CHWs can be seen as crucial resources in addressing myths and rumours, which are critical drivers of concerns on mpox vaccine safety and side effects; thus, bolstering vaccine acceptance and coverage. Community health workers (CHWs) play a central role in fostering partnership and collaboration between communities, health authorities and response actors for a harmonized approach to tackle mpox outbreaks.

Why is investing in regional and local manufacturing capacity essential to strengthening health security, and what is the EU doing to support it?

I interviewet fremhæver Martin Seychell, Deputy Director-General i DG INTERNATIONAL PARTNERSHIPS (INTPA), at global sundhed ikke kun handler om akutte kriser, men også om at skabe bæredygtig velfærd og stabilitet i samfundet.
Han peger på, at selvom økonomisk vækst finder sted i mange lande, betyder vækst alene ikke meget, hvis den ikke omsættes til bedre sundhed og livskvalitet.
Et centralt fokus er at investere i regional og lokal fremstilling af medicinske produkter - vacciner, medicin, diagnostik osv. - fordi erfaringen fra COVID-19 viste, at store dele af verden var ekstremt afhængige af import, fx underforsyning i sub-Sahara Afrika og Latinamerika.
Ved at styrke produktionen regionalt reduceres sårbarheden over for afbrudte forsyningskæder, eksportstop og manglende adgang under kriser.
Han beskriver, at EU's tiltag retter sig mod både at hjælpe partnerlande med at udvikle egne kapaciteter og at samarbejde globalt for at sikre mere retfærdig adgang til medicinske modforanstaltninger.
Endvidere fastslår han, at selvom globale sundhedstrusler (pandemier, smitsomme sygdomme) er akut vigtige, må indsatsen også omfatte det, der foregår "imellem kriserne" - opbygning af robuste sundhedssystemer og forebyggelse.

What is the situation with Mpox in the WHO European Region? Local transmission of clade I mpox marks a new phase for the strain in the European Region. Since the 2022 global mpox outbreak began, the WHO European Region has recorded more than 31,000 mpox cases. 2025 has seen a steady pattern of 200–300 new cases every month, mostly associated with clade IIb. However, clade I, historically confined to parts of Central Africa, is now being detected more frequently in Europe. To date, the Region has confirmed 72 clade Ib cases, alongside a small number of clade Ia cases and a few still under classification. Initially detected outside Africa in 2024 through cases linked to travel, in 2025, several European countries have now reported clade Ib infections in individuals with no travel history at all. This shift marks the first clear evidence of local circulation of this strain within the Region. Some of these infections have occurred among men who have sex with men (MSM), suggesting that clade Ib has begun to establish itself within the same sexual networks associated with the 2022–23 clade IIb outbreak. The virus requires very close physical contact to spread. Most people recover within a few weeks without medical treatment. Although there still remains uncertainty around the disease severity, infection with clade I can be more severe than clade II in young children, pregnant women, and those who are immunocompromised. WHO currently assesses the overall public-health risk as moderate for MSM and low for the wider population outside historically endemic regions. Sustained surveillance, rapid identification and isolation of cases, timely sharing of data, and community-centred communication on risk reduction behaviours—free from stigma—remain the backbone of mpox control. Vaccination continues to be an important tool, especially for those at higher risk. As we’ve seen repeatedly since 2022, supportive care, and non-discriminatory public health responses make all the difference. The emergence and local spread of clade Ib is a reminder that mpox continues to evolve, and our vigilance must evolve with it.

Deadline for public comments: 03 December 2025BackgroundMpox is a viral disease caused by the mpox virus, a member of the Poxviridae family - same family of viruses as smallpox. While it was discovered in 1958 in a captive monkey during a vaccine research in Denmark, it was first identified in humans in 1970 in the Democratic Republic of the Congo. In recent years, it has caused various outbreaks that spread outside Africa and warranted the WHO Director-General to declare two Public Health Emergencies of International Concern (PHEICs): one in 2022 during the global multi-country outbreak, and another in 2024 following a resurgence of cases in Central Africa with cross-border spread. These declarations underscore the growing international health threat posed by mpox and the need for sustained surveillance, rapid response, and equitable access to diagnostics, vaccines, and treatments. Laboratory testing is the basis for an efficient surveillance system, and therefore, the WHO is transitioning its interim guidance on diagnostic testing and testing strategies for mpox into a formal guideline.The guideline aims to provide evidence-based recommendations for testing and testing strategies for mpox, with the overarching goal of reducing transmission, minimizing morbidity and mortality, and strengthening preparedness and response capacities at national and global levels.Comments and perceptions brought to the knowledge of WHO through this process are an integral component of WHO’s Conflicts of Interest assessment policy to strengthen public trust and transparency in connection with WHO meetings involving the provision of technical/normative advice. Comments and perceptions will be carefully reviewed and treated confidentially. The comments will not be published and will be kept on record. WHO reserves the right to discuss information received through this process with the relevant expert, with no attribution to the provider of such information. Upon review and assessment of the information received through this process, WHO, in its sole discretion, may take appropriate management action in accordance with its policies. The purpose of this report is to raise awareness about the local transmission of clade Ib monkeypox virus (MPXV) among men who have sex with men (MSM) in countries previously unaffected or to date reporting only cases linked to travel. This report summarizes recent epidemiological developments, response activities, and the associated global public health risk. The second declaration of a public health emergency of international concern (PHEIC) for mpox was lifted on 5 September 2025. As both MPXV clades I and II and their subclades continue to circulate globally, leading to substantial outbreaks in African countries, WHO continues to advise emergency preparedness and response activities. Multiple modes of transmission underlie ongoing virus circulation, with sexual contact remaining the primary amplifier of transmission in most settings. Since 5 September 2025, several countries across four of six WHO regions have confirmed clade Ib MPXV infection in individuals with no recent travel reported (WHO African Region, Region of the Americas, the European Region and the Western Pacific Region), most of which are being detected among men who have sex with men, suggesting local transmission, particularly given that infections often manifest with few or no symptoms (paucisymptomatic or asymptomatic cases) leading to undetected onward transmission. Overall, the surveillance data in most countries is sufficient to detect and respond effectively to mpox outbreaks. However, thorough epidemiological investigation, contact tracing and implementation of public health interventions to control spread remain challenging. Mpox is known to resolve on its own over two to four weeks in most cases. However, timely access to quality healthcare is essential to identify, prevent and manage secondary bacterial infections and other complications. Individuals living with immune suppressive conditions remain at high risk of more severe mpox disease and death, most notably people living with undetected and/or untreated, uncontrolled human immunodeficiency virus (HIV) infection. Men who have sex with men with new and/or multiple partners remain at increased risk of clade Ib and also IIb MPXV infection. WHO assesses the public health risk posed by clade Ib MPXV to men who have sex with men as moderate and the risk to the general population as low in most countries.

In the coastal county of Kilifi, newborns were being hospitalised with what looked to be similar neurological symptoms - but diagnostic testing later showed that these infections were caused by the Chikungunya virus. The debilitating illness, which can cause fever and long-lasting joint pain, may also result in impaired consciousness and cognitive issues in some children. This year has seen an unprecedented surge of Chikungunya infections reported worldwide - including hundreds of cases being detected in Kenya. Nearly a decade on from the studies in Kilifi, a global research team are now working together to give doctors and scientists a better sense of just how widespread Chikungunya really is in the region. "The study has potential to yield significant public health benefits” says Professor George Warimwe (Nuffield Department of Medicine), including helping know who is most affected and where. These insights can help design future vaccine trials and guide decisions on how Chikungunya vaccine doses should be distributed.
